How healthy is your ASC?

16 questions. Two perspectives. One clear starting point for stronger performance.

Assess the latest three completed months.

Select the answer supported by your records. For each question, consider all listed elements and choose the level that best describes your center overall.

Yes / consistently = complete, documented, and sustained. Partly / inconsistently = gaps or uneven results. No / unknown = absent or not verified. Choose N/A only when the activity truly does not apply—not when data are missing.

How scoring works

Each applicable question earns 4 points for Yes, 2 for Partly, or 0 for No / unknown. N/A earns no points and is removed from that category’s possible points. Answer all 16 items; at least 4 must be applicable in each category.

Category score = earned points ÷ possible points × 100. Overall score is the equally weighted average of the two unrounded category percentages. Scores are rounded to a whole number and displayed on a 1–100 scale (a raw 0 displays as 1). Displayed category scores may differ slightly from the rounded overall average.

90–100: Strong foundation · 80–89: Generally healthy, with opportunities · 60–79: Improvement needed · 1–59: Significant improvement needed. A score below 80 triggers a recommendation to contact Surgery Solution Hunters. These are custom consulting score bands, not validated industry benchmarks.

Questions 1, 2, 5, and 6 also trigger a separate safety notice for Partly, No / unknown, or N/A. Financial performance never cancels a safety concern. N/A reduces assessment coverage, which is shown in the results.

A self-reported consulting screening tool—not a validated clinical instrument, accreditation determination, or financial valuation. A high score does not certify safety, compliance, or profitability. Targets should fit your specialty mix, acuity, contracts, and market. Answers stay in this page and are not submitted to Surgery Solution Hunters.
